Anal Skin Tags vs Piles: What They Are and When to Treat
An anal skin tag is a small, soft flap of extra skin at the edge of the anus. It is not a growth, it is not dangerous, and it is not the same thing as piles. Most people who have one do not know the word. They search for a lump near the anus that does not hurt, a bit of loose skin after piles, or a flap that makes cleaning difficult. If that sounds like your problem, this page is for you.
What a skin tag actually is
A skin tag is made of ordinary skin and a little fibrous tissue. It does not bleed on its own and it has no internal structure, unlike a pile, which is a cushion of blood vessels inside the anal canal. Tags are usually the same colour as the surrounding skin and feel soft and painless when touched.
Why they form
A tag is often what is left behind after something else has swollen and settled. Common starting points include:
- An external pile or a thrombosed pile that has shrunk back, leaving stretched skin.
- A healed anal fissure, where a small tag can sit at the lower end of the tear.
- Repeated straining, long-standing constipation or frequent loose stools.
- Previous anal surgery, or simply the skin stretching over time.
Skin tag or pile: how people usually tell the difference
Piles (hemorrhoids, or بواسير in Arabic) tend to come with bleeding after a bowel movement, itching, a feeling of fullness, or tissue that comes down and may need pushing back. A skin tag on its own usually causes none of that. It is more often a nuisance: hygiene difficulty, irritation from wiping, or a cosmetic worry. Many people have both at once, which is why an examination matters. Any new lump, bleeding or change in bowel habit should be looked at, not assumed to be harmless.
Do skin tags need to be removed?
Often, no. A tag that is painless and does not interfere with cleaning can simply be left. Skin tags alone rarely need proctology surgery. Removal is considered when the tag makes hygiene difficult, keeps getting irritated, or is part of a larger problem such as piles that are being treated at the same time. Anyone who tells you every tag must come off, or that one approach suits every patient, is selling, not advising.
Can skin tags be treated with day surgery laser proctology?
Yes, where removal is appropriate. Day surgery laser proctology means the procedure is done under local anaesthetic or sedation and you go home the same day. A tag can be removed on its own or alongside laser treatment for piles, so the skin is dealt with in one visit rather than two. Pain is usually minimal, though some soreness is normal while the small wound heals. Your surgeon decides what is suitable after examining you.
What to expect from a consultation
A good assessment looks at the tag, the anal canal and the surrounding skin, and asks about bleeding, pain and bowel habit. The aim is to separate a harmless flap from a pile, a fissure or a fistula opening, because each is managed differently. If piles are present, the options are explained honestly, including when laser is a good fit and when it is not. You can read more about laser treatment for piles.
