Piles in Young Adults: Why Your 20s Are Not Too Early

Medically reviewed by Dr Kerim Erdem Ulucay, Specialist General & Laparoscopic Surgeon (DHA-licensed) Last reviewed: 28 September 2026

Piles are not an older person’s condition. A large share of the people who come in with them are in their twenties and thirties, and most of them arrive surprised. They have read that hemorrhoids belong to a later stage of life, so when they notice bleeding after a bowel movement or a soft lump that comes and goes, they assume it has to be something else. Most people in this position search for piles at 25, hemorrhoids in young adults, or am I too young for piles. If that is roughly what brought you here, this page is about your problem.

Age is not the main risk factor

Hemorrhoidal cushions are normal anatomy. Everyone is born with them, and they help seal the anal canal. Piles develop when those cushions become congested, enlarge and begin to slip. What drives that is pressure, straining and time spent sitting, not the number of birthdays behind you. A twenty-six-year-old who lifts heavy while holding his breath, sits at a desk for ten hours and drinks very little water is under more anorectal pressure than a sixty-year-old with a high-fibre diet and a short commute.

The patterns that come up most often in younger patients:

  • long uninterrupted sitting – desk work, long drives, long-haul flights
  • heavy resistance training with breath-holding through the lift
  • low-fibre eating and chronic under-drinking, which the Dubai climate makes easy
  • phone time on the toilet, which turns two minutes of sitting into fifteen
  • repeated constipation, or the opposite, frequent loose stool
  • pregnancy and the pushing stage of childbirth

How piles usually present at this age

The most common first sign is bleeding after a bowel movement: bright red, seen on the paper or in the bowl, not mixed through the stool. Itching, a damp or mucus-stained feeling, and a soft lump that swells on straining and settles again afterwards are all typical. Marked pain is not. Uncomplicated internal piles rarely hurt, so when pain is the loudest symptom an anal fissure or a clotted external pile is often the better explanation. That distinction changes the treatment completely, and it cannot be made from a description alone.

Is laser piles surgery in Dubai suitable for someone in their twenties?

Age by itself does not decide this. What decides it is the grade of the piles, how much they interfere with daily life, and whether simple measures have had a fair trial first. Laser piles surgery in Dubai is generally considered for symptomatic grade two and three piles that keep bleeding or prolapsing despite sensible diet, stool and habit changes, and for patients who want to avoid the longer, more painful recovery of an open hemorrhoidectomy. It is a day-case procedure under local anaesthetic with sedation, and most people walk out the same day and are back at a desk within a few days.

It is not an automatic answer, though. Plenty of younger patients with grade one or early grade two piles improve enough with fibre, fluid and a shorter time on the toilet that no procedure is needed, and that deserves a genuine run first. Others have a grade or anatomy that suits a different technique better. Anyone who tells you one technique suits every patient is selling, not advising. The options are set out on the laser hemorrhoid treatment page.

Being young is not a reason to skip the examination

Rectal bleeding in a twenty-eight-year-old is usually benign. Usually is not a diagnosis. Inflammatory bowel disease most often declares itself between the late teens and the thirties, and colorectal cancer in the under-fifties has been rising for two decades. Persistent bleeding, a lasting change in bowel habit, unexplained weight loss, or a family history of bowel cancer are reasons for a proper look rather than a reassuring guess, and sometimes for a colonoscopy. Most people searching for piles treatment in Dubai simply want the bleeding to stop; the job of the consultation is to establish what is causing it first.

The other condition that turns up in this age group

If the problem sits higher, at the top of the buttock cleft rather than at the anus, a pilonidal sinus is worth considering. It is far more common between the mid-teens and mid-thirties than piles are, and it needs a different fix: laser pilonidal sinus surgery in Dubai closes the tract rather than leaving an open wound to heal over several weeks. The two get confused because both can show up as blood or discharge on underwear.

What is worth doing now, whatever the cause

  • aim for stool that passes without pushing: fibre from food first, a bulking agent if food is not enough
  • drink more than feels necessary, especially in summer
  • leave the phone outside the bathroom and keep visits short
  • breathe out through heavy lifts instead of holding your breath
  • do not keep using an over-the-counter ointment for months while bleeding continues

Seeing a piles specialist doctor in Dubai early usually means fewer options are off the table, not more. Grade one and two piles have more non-surgical routes open to them than grade four piles do.
